- We work with both self-managed and plan-managed NDIS participants
- We provide all required documentation for invoicing and plan tracking
Appointment Availability:
- Monday – Saturday availability
- Minimal wait times. We prioritise timely and consistent care
- Ongoing support, progress reports, and communication with plan managers if needed

We work closely with plan managers to ensure every NDIS participant receives high-quality, coordinated care with minimum admin hassle.
How to Refer a Client
Referring a participant to Rozelle Physio is simple:

What We Need From You
To get started, please provide the following:
- Participants full name and NDIS number
- Copy of their NDIS plan (goals section) if available
- Referring GP or Support Coordinator details (if relevant)
- Any reports or background information that might useful in guiding treatment
Ongoing Communication and Reporting
We value collaboration and keep plan managers updated throughout the care journey:
- Initial assessment report following the first session
- Progress reports provided at agreed intervals e.g. quarterly or before plan reviews
- Invoices and session notes available upon request
- Open communication for scheduling, funding queries, or updates to care needs
